In
Dar es Salaam, MDH supports the Government of Tanzania through the
Regional Government and Municipal councils of Ilala, Temeke, Kinondoni,
Ubungo and Kigamboni in the provision of comprehensive facility-based
HIV prevention, care and treatment services and program management. In
order to strengthen care and treatment of PLWHA in Dar es Salaam, MDH on
behalf of Ilala Municipal Councils seek to recruit Facility Based
Trackers for the HIV program for a short-term contract of two (2) months
from August to September, 2020OB TITLE: FACILITY BASED TRACKER (3)
REPORTING TO: Facility In-charge
Duties and Responsibilities
1. Identify patients who missed their clinic visits schedule timely and conduct telephone tracking.
2. Prepare and share with CBHS providers/peers/community tracker list of clients who were not found through the phone and those who don’t have telephone
3. Updates all tracking outcome into the CTC 2 card database with clinician/counselor and data officer.
4. Facilitate entry of tracking outcome into the CTC2 card and database with clinician/counselor and data officer.
5. Conduct weekly review and ensure all tracking outcome are updated and entered on CTC2 database
6. Ensure unknown clients are tracked timely and 70% returned to care monthly
7. Work with referral focal, counselor and data officer to confirm all transfer out in respective facility
8. Provide health education on importance of clinic adherence to the client and family
9. Supporting the patient adherence to medication and clinic visit schedules
10. Record and report tracking services daily, weekly, monthly and quarterly

11. Prepare summary report weekly, monthly and quarterly and submit to site manager and district retention and community officer.
12. Participate in data management and net loss analysis in respective facility and district.
13. Participate in QI activities that aimed to increase retention at facility level
14. Bring up any ideas or comments that may contribute to better system of patients follow up.
15. Preform any other duties as assigned by a line supervisor.
REQUIREMENTS:
• A Minimum of form four education, certificate computer, certificate or diploma in social work or community development or any other health related course.
• At least one (1) year working with HIV program.
• Good communication skills
• Committed to work and adheres to work regulations
• Maintains confidentiality in all aspects
• Analytical and problem-solving skills, multi-tasking and organizational skills. (Tracking skills is the other skills)

TB ADVISOR (1 Post)
Location: LINDI OFFICE
Reports to: REGIONAL TECHNICAL MANAGER
Duties and Responsibilities:
1. Lead and oversee planning, implementation and M&E of TB & TB/HIV services under the USAID Boresha Afya - Southern Zone Program, in her/his respective region.
2. Provide oversight and lead the implementation of key TB & TB/HIV program initiatives to address program, donor and national priorities - such as accelerating facility and community based TB case detection and management with special consideration for paediatrics, early detection and management of drug resistant TB cases and implementation of comprehensive TB/HIV collaborative services including 3Is initiative - in line with Tanzania national guidelines.

3. Actively participate in and provide technical assistance (TA) to Regional & Council Health Management Teams (R/CHMT), supported health facilities and community structures in planning and implementation of TB & TB/HIV programs and services.
4. Conduct regular need assessment, on TB & TB/HIV services provision in her/his region and organize efforts to address these needs -; including areas of capacity building and health system strengthening - in collaboration with the R/CHMT.
5. Ensure timely collection and submission of TB & TB/HIV services data as well as support strengthening of the related M&E systems - in collaboration with the M&E team.
6. Lead efforts to robustly analyze and utilize TB & TB/HIV services data to inform plans, priorities and resource allocation.
7. Lead efforts to identify gaps in TB & TB/HIV services provision, design and implement Continuous Quality Improvement (CQI) initiatives to address the gaps identified.
8. Organize and support routine meetings with key project stakeholders to share, monitor and evaluate program progress towards assigned targets.
9. Prepare timely quarterly and annual narrative reports, best practices, lessons learned and other relevant documents on TB & TB/HIV services as assigned by line manager.
10. Support and advise the line manager on all TB & TB/HIV services issues and perform other relevant duties as assigned by the line manager.

Requirements: Experience, educational and professional qualifications
a) Medical Doctor degree
b) Master’s degree in Public Health or related field is an added advantage.
c) At least 3 years of experience in managing public health programs/services.
d) At least 2 years’ experience in designing, implementation, and management of TB-related programs.
e) Excellent written and oral communication and presentation skills in English and Kiswahili.
f) Ability to work under pressure and stringent deadlines.
